At Palmdale Pediatric Center, the visit includes a review of relevant history and an age-appropriate physical exam. Families should bring every form provided by the school, team, or program.

The clinician may ask about prior injuries, current symptoms, medications, activity, and health history. The exam is intended to identify questions that may need follow-up before a form can be completed. A sports physical is not a substitute for every part of ongoing preventive care, so the team may also discuss whether a well-child checkup is due.

Scheduling before a deadline gives families time to gather records and address any follow-up that may be needed. Call the office with the form requirements and desired timing. Please complete the parent or guardian portions of forms before the appointment whenever possible.
